Images scaled with mipmap filtering using ANGLE on Intel q35 chipsets display as black boxes

    Description

      Under qt 5.3 and 5.4, when using Angle for rendering, qml Image elements with mipmap filtering sometimes display as blackboxes.

      I have seen this on a machine with an Intel(R) Q35 Express Chipset, running driver version 8.15.10.1930.

      Attached a test project showing the problem as well as some log output with qt.qpa.gl logging and QSG_INFO=1.
