When ClearType is enabled, text rendered in a graphicsview with QGraphicsItem::DeviceCoordinateCache enabled appears very broken, as seen with QGraphicsEffect on a QGraphicsSimpleTextItem. A demonstration of this can be seen at http://i44.tinypic.com/ncgolw.png or with the attached testcase. The conditions used here seem to be ideal for exaggerating the problem, but it is not specific to any font, font size, or color combination. From a zoomed comparison of the text (http://i42.tinypic.com/25twbwi.png), the broken text appears to be rendered identically to cleartype, except that it is done entirely in shades of gray.
