Allow creating only .ui.qml file

      Currently, when a user creates a new UI Form file, QDS generates .qml and .ui.qml files.

      Sometimes we only want to create .ui.qml file, so it should allow a user to choose whether he wants to only create .ui.qml or both .ui.qml and .qml files.
