Consider root cause analysis (RCA) for different development phases based on the experiences (e.g. design phase)
And of course, based on the bug reports in the testing phase.
Use for example 5 Why methods to identify the root cause. And then solve those root causes instead of reacting to the symptoms. (the Qt development can afford to fix the bugs afterward, but the QSR core team has limited resources)
