Adding a QT_LINUX_ACCESSIBILITY_ALWAYS_OFF to disable accessibility for a specific app

    Description

      It should be could to be able to control the sending of ATSPI events from a central place (via env variable).

      Adding a environment variable QT_LINUX_ACCESSIBILITY_ALWAYS_OFF that disable the accessibility would be useful for this.

      For now if Qt was built with accessibility, then even if QT_LINUX_ACCESSIBILITY_ALWAYS_ON is not set and QT_ACCESSIBILITY is set to 0, the accessibility is still on.
