Investigate Qt::AA_PluginApplication hardening

We may not respect Qt::AA_PluginApplication as much as we should.

For example, on macOS we do some initial checks for Qt::AA_PluginApplication when setting up the global menu bar, but not later on, so a plugin is able to affect the hosting app menus. That probably not what we want.
