Allow user to add non-all-lower-case HTTP header

There are some legacy non-standard systems that require the HTTP headers to have the initial letters of their names capitalized. For example, "Content-Type" is accepted but "content-type" is considered bad.

Maybe we can add a switch for that since right now, we are doing normalization regardless.
