QGLWidget - does not render when opened from second monitor

QGLWidget works on the main monitor but comes up in single color on the second monitor.

Included a test case to reproduce.

Step to reproduce:
-Launch the application from the main monitor, the background is green as expected
-Launch the application from the second monitor, the background is black (problem)
-Launch the application from the main monitor and drag it to the second monitor, the background is and stays green
-Launch the application from the second monitor and drag it to the main monitor, the background is and stays black

Each monitor can be used as the main monitor and it works as expected there.
Each monitor can be used as the second monitor and the problem arises there.
