Remove a logically possible case where an attribute can be written without a start tag being present

      I doubt this ever occurs, but it is at least logically possible in one case in the DITA XML generator, where an attribute can be written to the XML file without there being a start element for the attribute to appear in. This crashes on the Windows platform, but not on the others.
