Application's Icon on Mac should not show in title bar.

Qt applications on the mac display the application's icon next to the title in the title bar of their widgets. This does not match native behavior.

On Mac the windows do not automatically take the icon from the application, instead the apple interface guidelines allow occasional icons (for example for documents or folders)

As Qt strives to resemble native applications as close as possible Qt applications should respect this native "default icon free" look and also still allow the developer to set a icon on a window explicitly if desired.
