[PATCH] Load all available pixmaps from an icon file.

Some image formats can contain multiple images, like the Microsoft .ICO format. The first patch fixes the problem that QIcon would only load the first icon from a file. The second patch fixes Qt Assistant when used as a custom help viewer, which would not read all sizes of the application icon specified in the .qhcp file. Now application icons look good in both Vista and Windows 7, provided the files contain multiple icons of various sizes. The two patches are independent of each other, and maintain binary compatibility.
