QTextDocument::contentsChange() bad parameters

      Various actions cause bad values for charsRemoved and charsAdded.

      copy/paste a single character into the top-left-most position of a QPlainTextEdit. charsRemoved and charsAdded are set to non-nonsensical values. Problem only seems to occur at top of document.
