qdoc tries to autolink symbols that begin with "__"

      There are several symbols that begin with two underscores in the documentation. qdoc tries to autolink these symbols, but they shouldn't be autolinked. In at least one case ("_FILE_"), qdoc successfully autolinks the symbol, but the link goes to the wrong place.
