QMenu: submenus are not highlighted correctly on Mac/Cocoa

When having sub-menus in a widget/dialog or a context menu, i.e. not in the native menubar, then several sub-menu items can be highlighted at the same time when hovering over them.
Also, the selection does not clear when moving outside the item.
