[Mac OS X 10.10] Multimedia 'customvideoitem' example crashes at startup

    Description

      The example application 'customvideoitem' (Qt/Examples/Qt-5.4/multimediawidgets/customvideosurface/customvideoitem) fails unexpectedly at startup.

      Tested with the offline installer qt-opensource-mac-x64-clang-5.4.1_2014-12-16_00-46-17-91.dmg, but most probably can be reproduced with: http://download.qt.io/snapshots/qt/5.4/5.4.1/2015-01-07_7/ qt-opensource-mac-x64-clang-5.4.1_2015-01-07_20-01-53-7.dmg
