Unification of example licenses and adding commercial option to BSD header

Details

    Description

      Most of the examples are BSD licensed but some individual files (e.g. http://doc.qt.io/qt-5/qtquick-shadereffects-shadereffects-qml.html) have LGPL license header. We should harmonize the usage of the license headers in all examples.

      Furthermore we need to add "Commercial License Usage" option to the BSD license header (same as in LGPL license header).
