qdoc: Empty include statement for \headerfile documentation

    Description

      The generated html file for a \headerfile contains an empty #include <> statement. e.g.

      https://doc-snapshots.qt.io/qt5-5.13/qtglobal.html

      Also, title of the page is prefixed with the string 'Header', which sometimes makes the title a bit nonsensical.
