qdoc: Handle missing .index and/or comments file in generate phase

      Generate phase should exit with a suitable error message if either if the index/comment file for the primary tree cannot be found.

      To test, run 'make prepare_docs', delete the generated index file, then run 'make generate_docs' - weird things happen.
