qdoc: Improve formatting of subtitles on class reference pages

    Description

      There are a few issues with the output of subtitles (which are auto-generated for classes):

      • The </br> at the end of a subtitle is unnecessary
      • We have no styling (CSS) for subtitles
      • In some cases, the subtitle is treated as a paragraph of text which doesn't look too good and is unnecessary
      • We should link to each parent in the full name - class Foo::Bar::Baz
