QToolbar rendering broken

      QToolbar rendering is completely broken in QT versions > 5.9 when setUnifiedTitleAndToolBarOnMac is enabled. See the attached video for details.

      The application is GPXSee (https://github.com/tumic0/GPXSee) build with Xcode 10 on OS X 10.13. (OpenGL is of course disabled, with OpenGL enabled the toolbar is completely black, but we all know you have made this a feature years ago).
