Review and clean up the QTDS driver

      Clean up the QTDS driver and test it with a Sybase server. Make relevant updates the QTDS documentation.

      The driver has code both for MS SQL Server and Sybase. MS SQL Server no longer use TDS, but rather ODBC, so we only need to support Sybase.

      Note that the driver uses DB-Library which is no longer developed (no new features added, only bugfixes). The driver should really be ported to CT-Library, but this might not be worth the effort.
