qdoc: Improve auto-generated navigation for table-of-contents pages

      Given a \page with a nested list structure defining a table of contents, QDoc has the capability to automatically set the \nextpage and \previouspage links for each page in the TOC.

      The TOC hierarchy should be also reflected in the generated breadcrumbs for html output, which currently does not happen.
