Menu item should not appear/disappear dynamically. Enable/disable instead

Any menu item should not appear/disappear dynamically (unless the menu differs a lot, like totally two different menus). Instead, when disabled, they can be dimmed.

I don't explore all UI's but the first menu item I found to appear/disappear dynamically is "Remove All Expression Evaluators" in "Locals and Expressions" view window.

In my experience to add two variables to watch, I tried to click on "Insert new expression evaluators" twice. However, at the 2nd click, the menu item changes. "Remove All Expression Evaluators" dynamically appears at this time, right at the same position, which causes me to wrongly click on it. This removes my first one (and without a confirmation dialog, which is already documented as a bug)
