QT Designer form class dialog fails

    • Test on a build made on Mac OS X 10.10 with QT 5.4.1 and on a build made on Windows 7 x64 (MSVC2013 x86) with QT 5.4.1

    Description

      While creating a new QT Designer form class the Summary page from the wizard fails to display any information related to the newly created file.

      And if the button finish is clicked the QT Creator crashes!

      No information available for any of fields:

      • add to project is empty with no posibility to select some value
      • same for for add to version control

      Also there is no information for "Files to be added in".

      The problem is not existent on 3.4 branch only on master.
