Offer file renaming when refactoring class

      Sometimes when refactoring it can be necessary to change the name of a class.
      It would be handy if QtCreator could detect a class name change, and offer to
      rename the cpp and h files accordingly, and to change any #includes that refer
      to those files.
      If this could also be linked into source control, it would make this
      refactoring very easy.
