No update on CMake tool add/remove

      If one adds a new CMake tool using CMakeProjectManager::CMakeToolManager::registerCMakeTool, the view Tools->Options...>Build & Run>CMake does not show the newly added entry.

      If one removes a CMake tool using CMakeProjectManager::CMakeToolManager::deregisterCMakeTool, that CMake view is not updated. Moreover, it is not removed from the settings (cmaketools.xml).
