Dialog for crashed application appears twice

      1. Go to "Tools" -> "Options…" -> "Debugger" -> "General".
      2. Check "Use Qt Creator for post-mortem debugging"
      3. Click "OK" and quit Creator.
      4. Start an application independently of Creator.
      5. Crash this application.
        A dialog will ask you: 
        The application "" (process id 7280) crashed. Would you like to debug it?
      6. Click "Cancel".
        The same dialog will appear again.
