Find References to Symbol Under Cursor shows duplicates

The Find References to Symbol Under Cursor feature started showing duplicates from qtc 10. It's not reproducible everytime, but when it starts happening it happens each time.

I have tried primarily on class data members. I'm gonna to attach 3 pictures of this bug, there is clearly visible the pattern whats up.

It only seems to happen for the results (or occurrences) from the first file, this file shows up again and the occurrences are displayed 2 times, so the results from the first file show up 3 times, all subsequent search results from all other files are fine.
