Consolidate Qt Creator GUI for managing QML code model/language server

Currently, "Tools" > "QML/JS" > "Reset Code Model" is only applicable to Qt Creator's built-in code model. If qmlls is enabled, that menu item has no effect (https://bugreports.qt.io/browse/QTBUG-114697?focusedId=728427&page=com.atlassian.jira.plugin.system.issuetabpanels:comment-tabpanel#comment-728427 ) but it remains active and clickable, which is confusing.

Also, qmlls controls are only accessible via the tiny text-less tool button (https://doc.qt.io/qtcreator/creator-coding-navigating.html#managing-language-servers ).

Suggestions

• (Short-term) Disable the "Reset Code Model" button if qmlls is active
• (Long-term) Make qmlls controllable via "Tools" > "QML/JS"
