Valgrind error dialog issues

      I tried to run my application with the new analyzer tools in creator. The application was built in release and valgrind was installed and working (via the command line). When I tried to use the memory analyzer, I got this dialog.

      1) Message is incorrect - the application is running in release.
      2) The message is repeated three times. Run. In. Release. I get it!
      3) Not a sheet.
      4) Buttons are the wrong way around. Cancel, the default button, should be on the right.
      5) Application icon missing.
      6) Dialog is resizable.
      7) Close and maximize buttons are enabled but this is not the convention for model dialogs on OSX (would be fixed by 4).

      (The developer should re-read Qt's own documentation on MacOSX development)

      This output was hidden in the log --tool=callgrind: No such file or directory so I'm guessing somethings not configured correctly. Still the dialog needs fixing.
