Attaching a file to a comment deletes the entire comment text (and results in 2 email notifications)

When you enter a comment and then use the attachment button from the toolbar to attach a (new) file, 2 undesirable things happen:

1) your comment is deleted without warning

2) an email notification is sent, apparently before you even post the comment.

I'm mentioning these in the same ticket because I think there might be a cause-and-effect relationship between them.
