Finally I got an Art Pen for the Intuos 4 to be able to test this. When I hold it straight-up, the rotation should be around 0, but it's about 270. When I rotate it clockwise the number decreases; it should increase. When I rotate it to the left, it increases, and when it's almost upside-down, it goes past 360 and wraps around to 0 again.

Testing on Windows 11 with the latest Wacom drivers.
