CMake server mode adds files multiple times

    • Tested with CMake 3.7.2 and CMake 3.8-rc4 on Windows.

      In CMake server mode source files and header files are shown multiple times in the project tree.

      Additionally, include paths for headers are wrong in the code model.

      Simple test project is attached.
