Design tab is missing menu bar and mouse position is offset.

The Qt Quick Designer is missing the menu bar at the top (see attached image). Additionally, all of the controls on the screen are offset such that the mouse cursor and click location are offset (that is, where the mouse clicks is about 5-10 pixels above the cursor). The text is extremely blurry.

This behavior exists not just with Qt Quick Designer but also with any applications built with Qt Quick 2 controls. The designer is almost completely unusable in this state.

This occurs with a brand new Qt 4.4.0 Enterprise installation.
